However, it’s genuinely two questions wearing one coat: how long does the visible improvement last, and how long does the product itself stay in your face? For years, everyone assumed those were the same. Then radiologists started looking.

In 2024, a review of 33 patients who underwent facial MRI found hyaluronic acid filler visible in the mid-face of every single patient at two or more years after injection. Twelve patients hadn’t been injected in over five years — the product was still there. The longest persistence documented was fifteen years (PRS Global Open, 2024). The authors’ conclusion was blunt: the conventional assumption that filler metabolizes within 3-12 months underestimates its residence time, and refill protocols should be reconsidered.
Here’s the reconciliation, and it matters more than either number alone: your result fades before the filler is fully gone — and the filler is often still there long after you stop seeing it. The gel loses water, shifts, integrates with tissue, and stops providing crisp correction while physically persisting. In my experience, this single fact reframes everything about maintenance.
Why? Because if a “top-up” goes on top of product that never actually left, you’re not restoring a result — you’re stacking. Do that on a schedule for a few years and you arrive at what the literature now formally calls overfilled face syndrome, a recognized clinical entity with its own classification framework as of 2025 (narrative review and classification). The heavy, pillowy, “done” look isn’t caused by one bad injection. It’s caused by arithmetic.
